It appears that the chair shot to the head from Shawn Spears to Cody Rhodes was intended but Cody was not supposed to bleed. According to PWInsider.com, something went wrong when the top of the chair caught Cody accidentally.
Cody legitimately needed twelve staples and was still bleeding backstage while working behind the scenes as you can see in the photo below:
